Creatine: What the Evidence Actually Supports, and What New Zealand Rules Say
Creatine is one of the most researched sports supplements, but tubs still oversell forms, loading protocols and health claims. Here is what the evidence supports for training, how New Zealand's food and sports-food rules apply, and what to check on the label before you buy.

A five-gram scoop of creatine powder looks small beside a shaker bottle. Its effect, if it has one, is not a burst of energy like caffeine. Creatine raises the amount of creatine stored in muscle over time, which can help the body remake energy during repeated, hard efforts. That distinction explains why it has a better evidence base for some training goals than for a general promise to feel fitter.
Creatine is also an easy category to overcomplicate. Tubs advertise hydrochloride, buffered blends, rapid absorption and “no loading.” In New Zealand, the more useful questions are simpler: what is in the scoop, what does the label ask you to take, what evidence matches your goal, and does a supplement make sense in the first place? This guide separates those questions from marketing claims. It is general information, not a substitute for advice from a clinician, pharmacist, registered dietitian or sports dietitian.
What creatine is doing in the body
Creatine is a naturally occurring compound made from amino acids. The body makes it in the liver and kidneys, and food contributes some too, particularly meat and seafood. The 2017 International Society of Sports Nutrition position stand reports that roughly 95 percent of the body’s creatine is in skeletal muscle. Much of it sits there as phosphocreatine.
During a short, demanding effort, muscle needs to regenerate adenosine triphosphate, usually called ATP, quickly. Phosphocreatine can donate a phosphate group in that process. This is why creatine is most relevant to repeated high-intensity work: hard sets of resistance exercise, sprints, jumps, and similar efforts with brief recovery. It is not a stimulant, and taking it does not replace sleep, food, fluids, skill practice or a training plan.
The effect is indirect. A person whose muscle stores rise may be able to complete a little more high-quality work across training. Over weeks and months, that extra work can contribute to training adaptation. It does not mean a single dose produces a visible change that afternoon. It also does not mean every sport, every workout, or every person responds in the same way.
The 2021 ISSN review describes creatine supplementation as relatively well tolerated at commonly studied intakes of 3 to 5 grams a day, or about 0.1 grams per kilogram of body mass per day. Those figures describe research protocols; they are not a personalised prescription.
Muscle does not store creatine indefinitely. Once intramuscular concentrations reach a ceiling, taking more does not push the store further; the surplus is excreted rather than banked for later use. That ceiling is part of why very large recreational doses are pointless rather than simply cautious. It is also why consistency across weeks matters more than an occasional large dose taken before a big session, since the store rises gradually rather than topping up in a single sitting.

The performance claim with the clearest support
The strongest, most consistent evidence is for repeated, high-intensity exercise and resistance-training outcomes. The ISSN position stand says supplementation increases intramuscular creatine concentrations and may help explain improvements in high-intensity exercise performance and training adaptations. Mayo Clinic similarly states that creatine may help athletes do more work during repeated short bursts of high-intensity exercise, including weightlifting and sprinting, and that resistance training plus creatine can lead to greater gains in strength and muscle size.
That wording matters. “May help” is different from a guarantee of a personal-best lift, a certain number of kilograms of muscle, or a change without training. A recreational lifter who follows a sensible programme may be a closer match to the research than someone hoping creatine will compensate for irregular meals, very little sleep and sporadic workouts.
There is no sensible reason to treat creatine as a test of commitment. Many people train well without it. For someone who does use it, the productive comparison is not creatine versus doing nothing. It is creatine alongside an adequate programme versus the same programme without it. The programme still does most of the work.
Useful reality checks include:
- A result measured in a study average may not appear in every individual.
- Water held in muscle can increase body mass early, so scales cannot separate water from longer-term changes in lean tissue.
- Endurance events dominated by long, steady output are not the clearest use case, although some sports combine endurance with repeated surges.
- A formula promising fat loss, hormones, testosterone or effortless transformation is making claims that go beyond the core creatine evidence.
Food, muscle stores and who may notice more
The body both produces creatine and obtains it from food. The ISSN position stand notes that red meat and seafood are dietary sources and that the body replaces creatine daily to maintain its usual stores. People who eat little or no animal food may begin with lower muscle creatine stores. That does not make supplementation mandatory, but it is one plausible reason some people may see a larger increase in muscle creatine after taking it.
Diet is not a purity contest. A person can meet protein and energy needs through many food patterns, including vegetarian and vegan diets, with appropriate planning. Creatine is a separate question about a compound that food and the body already provide. It should not be marketed as proof that a vegetarian diet is inadequate, or as a reason to neglect protein, carbohydrate, iron, vitamin B12, calcium or total energy intake.
For athletes, the first audit is practical. Are meals sufficient for training volume? Is recovery reliable? Is there a medical reason for fatigue, poor performance, weight change or gastrointestinal symptoms? The Sport Integrity Commission Te Kahu Raunui starts its supplement decision guide with a food-first approach and asks athletes to consider advice from a registered nutritionist, dietitian, GP or sports doctor. That is a more useful starting point than comparing tubs online.

A food-first approach does not prohibit supplements. It asks whether the proposed supplement has a defined job, credible evidence and a sensible risk profile in the context of the individual’s diet and sport.
Monohydrate, micronised powder and newer forms
Creatine monohydrate is the form used in much of the foundational research. “Micronised” generally describes particle size, intended to help powder mix more easily. It does not turn the ingredient into a different proven mechanism. If a label lists creatine monohydrate and a serving size, a buyer can at least identify the form and the amount being sold.
Other forms may be presented as premium upgrades: creatine hydrochloride, buffered creatine, ethyl ester, nitrate combinations or multi-ingredient blends. Better solubility in a glass, a smaller scoop or a novel name does not by itself establish a superior training outcome. The 2021 ISSN review specifically examined whether forms other than monohydrate are similar or superior and describes monohydrate as the form with the substantial body of evidence behind common use.
This is not an instruction to dismiss every new product. It is a reason to ask a higher standard of proof for a higher price. A label that hides the amount of each ingredient behind a proprietary blend makes the comparison harder. A product that combines creatine with stimulants, herbs or other substances creates more variables, more possible interactions and more reasons to read the label closely.
The practical label question is: how many grams of creatine are provided by one stated serve, and which form is it? A second question follows: does the product include other active ingredients that matter to you? If the answer is unclear, a plain product with a complete ingredient list is easier to evaluate than a dramatic front-of-pack claim.

Dose, loading, timing, and what early weight gain can tell you
Two approaches commonly appear in research and practice. A loading protocol uses a larger daily amount for several days, divided into multiple doses, before moving to a lower maintenance intake. A steady approach uses a smaller daily amount from the start. Loading can raise muscle stores faster. It is not required for those stores to rise over time.
The ISSN literature describes commonly studied maintenance intakes around 3 to 5 grams per day and discusses loading strategies in the research context. The right response to a label is not to copy a social-media routine blindly. More is not automatically better, and a large dose can be less comfortable for some people. Anyone considering a protocol beyond ordinary label directions should discuss it with an appropriately qualified clinician or sports dietitian.
Timing is usually less important than consistency for this particular supplement. Creatine works by building muscle stores rather than creating a narrow pre-workout window. Taking it at a time that fits a consistent routine can be more useful than repeatedly changing the schedule. That might be with a meal, after training, or another regular point in the day. If a product causes stomach upset, taking it with food or using divided doses may be worth discussing with a clinician rather than assuming discomfort must be tolerated.
A few sensible boundaries:
- Follow the product directions unless your healthcare professional gives individual advice.
- Do not treat a loading phase as compulsory or as a shortcut to results without training.
- Do not double up after a missed day.
- Keep water, food and recovery practices ordinary and adequate. Creatine does not cancel the need for them.
- Stop and seek advice for symptoms that concern you, especially if they are persistent or severe.

One reason creatine generates confusing stories is that body mass can rise soon after starting it. Creatine increases the amount of water held inside muscle cells. This is different from proving that a person has gained body fat, and it is also different from proving that every kilogram on the scale is new muscle tissue.
For an athlete in a weight-class sport, a runner tracking race mass, or anyone whose body weight is clinically relevant, that distinction has consequences. A change on the scale needs context. Look at the training goal, intake, symptoms, performance measures and the time course, rather than assigning a moral value to the number.
The early change can be inconvenient if someone has a near-term weigh-in. It can also be harmless in a different context. The point is not to promise a specific amount of weight change. Individual responses, starting stores, food intake and training differ too much for that. The point is to prevent a predictable misunderstanding: a scale is not a laboratory measure of muscle growth.
Photos can be misleading for the same reason. Lighting, posture, hydration, glycogen, food volume and timing all affect appearance. A simple training log with exercises, sets, repetitions and load is more informative than daily mirror comparisons. If the goal is health rather than competition, the measure that matters may be whether the routine is sustainable and appropriate, not whether a supplement creates an immediate visual result.
Kidneys, creatinine and the safety conversation
Creatine is often discussed as if one sentence can settle every safety question. It cannot. The evidence in healthy adults at recommended intakes is reassuring, while people with kidney disease or other relevant medical conditions need individual clinical advice.
Mayo Clinic states that studies in healthy people taking recommended doses have not found harm to kidney function, while research in people with kidney disease is limited. Its advice is appropriately cautious: people with kidney disease may want to speak with their healthcare team before using creatine. That is a better standard than either declaring creatine universally dangerous or declaring it safe for every person in every circumstance.
Lab interpretation adds another layer. Creatine is metabolised to creatinine, a compound commonly measured in blood tests used to assess kidney function. A higher creatinine result does not automatically mean that creatine has damaged kidneys, but it is information a clinician needs when interpreting the full picture. Tell the clinician about creatine use, the amount, the product and when you started. Do not stop prescribed medicines or self-manage a worrying test result based on a forum post.
Extra caution and professional advice are sensible for people who:
- Have known kidney disease, reduced kidney function or a history of kidney problems.
- Are pregnant, breastfeeding, under 18, or considering creatine for a child. Evidence and clinical context differ from the adult sports research.
- Take medicines or have conditions where fluid balance, kidney function or laboratory monitoring matters.
- Have unexplained fatigue, swelling, changes in urination, ongoing nausea or another symptom that warrants assessment.
The goal is not alarm. It is to avoid borrowing evidence from healthy adults and applying it automatically to a person who was not represented by that evidence.
Regulation in New Zealand depends on the product
A creatine product is not regulated only by the word “creatine” on the tub. In Australia and New Zealand, FSANZ says sports supplements may be regulated as a food or a medicine depending on the ingredients, claims and product form. That means marketing language matters as well as composition.
FSANZ explains that formulated supplementary sports foods have specific label requirements. The label must identify the food category, provide recommended quantity and frequency of intake, state recommended daily consumption, and carry statements that they are not a sole source of nutrition. It must also state that the product is not suitable for children under 15 years or pregnant women and should only be used under medical or dietetic supervision.
New Zealand’s Ministry for Primary Industries explains that the New Zealand Food (Supplemented Food) Standard is a New Zealand-only standard for foods represented as having substances added or modified to perform a physiological role beyond a simple nutritive requirement. The user guide says supplemented foods sold in New Zealand must also comply with relevant legislation, including the Food Act 2014 and Fair Trading Act 1986.
For a shopper, this is a reason to examine the actual package rather than assuming every powder follows one identical rule. Read the ingredient list, the nutrition information where applicable, the serving direction, warning statements, importer or supplier details and the claims. A flashy overseas product page does not replace the information required for the product you are actually buying and consuming in New Zealand.
Labels: the five checks that prevent most confusion
A good label-reading routine is quick. It does not require memorising biochemistry.
- Identify the ingredient. Look for the exact creatine form, such as creatine monohydrate. Do not rely solely on a front-panel name.
- Find the amount per serve. Check how many grams of creatine the stated serve supplies. Check whether the scoop size matches that serve.
- Read the whole ingredients list. A plain powder and a pre-workout blend are different products. Caffeine, sweeteners, herbs and other additions may change who should use it and when.
- Read directions and warnings. They are part of the product, not fine print to ignore. Sports-food labels may carry specific age and pregnancy warnings.
- Check the seller and product traceability. Batch number, supplier information and a legible label help if there is a quality question or recall.
This approach also exposes vague claims. “Clinically dosed” is not a regulated explanation of what is in a scoop. “Advanced matrix” may not identify any amount at all. If the label does not let you determine the ingredient and quantity, it does not give you enough to compare the product against the evidence.
A third-party quality-testing claim can be useful, but it is not a licence to stop reading. Check what the certification covers, whether the exact product and batch are included, and whether the seller’s claim can be verified. It also does not convert a product into medical advice.

Athletes: permitted substance does not erase supplement risk
Creatine itself does not appear on WADA’s Prohibited List. But that narrow fact is not the same as an approval of any product sold as creatine. The World Anti-Doping Agency updates its list at least annually, and athletes are responsible for checking the current rules that apply to their sport.
The Sport Integrity Commission makes the wider point plainly: it cannot approve any supplement product or its use because it cannot be 100 percent sure a supplement is free from banned substances. The guide says athletes can test positive because of contaminated supplements and remain responsible for what is found in their sample. It also notes that inaccurate labelling and factory cross-contamination make it difficult to know what is really in a product.
This is especially relevant to products that bundle creatine with stimulants or “performance” ingredients. The appeal of an all-in-one formula is convenience. The trade-off is reduced transparency and a larger contamination or labelling-risk surface. A single-ingredient product does not eliminate risk, but it is easier to inspect than a long proprietary blend.
Competitive athletes should use their sport’s current education and testing resources, keep product and batch records, and seek qualified sports-dietetic advice. No influencer, retailer or supplement brand can take over strict-liability responsibility for an athlete’s sample.
A decision that fits the person, not the hype
Creatine monohydrate has a clear place in sports-nutrition research: it can support repeated high-intensity training when used alongside a well-built routine. That is enough. It does not need exaggerated claims about every health goal, and it does not need to become a default purchase for every gym member.
Before buying, name the goal. If it is strength or repeated high-intensity performance, compare a transparent product with the evidence and your training plan. If it is low energy, poor sleep, unexplained changes in weight or a health concern, a supplement tub is often the wrong first diagnostic tool. Speak with a healthcare professional instead.
For most consumers, the decisive information is on the back of the pack: the form, grams per serve, full ingredients, directions, warnings and supplier details. For athletes, add contamination risk and current anti-doping advice. For anyone with kidney disease, pregnancy, breastfeeding, adolescence, medication use or a medical condition, add an individual conversation with a qualified professional before starting.
Creatine is not magic powder. It is a well-studied compound with a fairly specific training role, a label that still needs reading, and limits that marketing should not be allowed to blur.
The available creatine research is large by supplement standards, with hundreds of published studies, but that volume does not mean every question has a firm answer. Sex-specific research is thinner than research in men, and the 2021 ISSN review notes ongoing work on how the menstrual cycle and hormonal contraception may interact with creatine response. Studies in adolescents and children are limited to specific clinical contexts, such as diagnosed creatine-synthesis disorders, rather than general sports use in minors.
Long-duration safety data is reassuring for years-long use in healthy adults at commonly studied doses, but it is not infinite. A person planning years of continuous use, rather than cycles tied to a specific training block, is a reasonable candidate for periodic check-ins with a GP or sports physician, particularly if other supplements, medicines or health conditions are part of the picture. None of this is a reason for alarm. It is a reason to treat “generally safe” as a summary of the current evidence rather than a permanent guarantee that applies to every future product formulation or every individual circumstance.
